    All the MAST Intake forms (white pages) (every 3 years) and the Nutrition Risk forms (blue pages) (every year) that were distributed during the last 2 weeks are all due back as soon as possible. Both forms are required to be completed by the Agency on Aging for the state database of meal clients at each senior center. Returned data from these 2 forms help in providing funding from the state to the Harlem Senior Center for our meal program.     If you are not one of our “regular” meal clients (meaning you eat, or get meals delivered, at least 3 or more days a week) you must call in BEFORE 10:00 am to let us know that you will be wanting a meal, whether in the Center or delivered.
